Ils sont \u00e0 l&rsquo;origine de la courbe du potentiel d&rsquo;action des cardiomyocytes. Les flux potassique et sodique peuvent \u00eatre affect\u00e9s par des maladies ou des m\u00e9dicaments (maladie d&rsquo;Addison, syndrome d&rsquo;obstruction f\u00e9line, my\u00e9lome, t\u00e9tanie puerp\u00e9rale, diur\u00e9tiques, digoxine, anti-arythmiques, etc&#8230;)<\/p>\n<figure id=\"attachment_737\" aria-describedby=\"caption-attachment-737\" style=\"width: 319px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-737 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/potentiel-action2.jpg\" alt=\"canaux trans-membranaires, potentiel d'action et aspect ECG\" width=\"319\" height=\"299\" \/><figcaption id=\"caption-attachment-737\" class=\"wp-caption-text\">Potentiel d&rsquo;action d&rsquo;un myocyte et sa traduction sur l&rsquo;\u00e9lectrocardiogramme<\/figcaption><\/figure>\n<h3><strong>hyperkali\u00e9mie:<\/strong><\/h3>\n<p>Les principales alt\u00e9rations portent d&rsquo;abord sur la phase 3 de la courbe du potentiel d&rsquo;action\u00a0 cellulaire (Pa), c&rsquo;est \u00e0 dire sur la repolarisation. La premi\u00e8re modification portera donc sur la morphologie de l&rsquo; onde T.<\/p>\n<p>Puis, lorsque la kali\u00e9mie augmente, ce sont les phases 1 (d\u00e9polarisation) et 4 (potentiel de repos) qui sont modifi\u00e9es.<\/p>\n<p>R\u00e9percussions \u00e9lectrocardiographiques:<\/p>\n<ol>\n<li>Cela a pour cons\u00e9quences un ralentissement de la conduction et une r\u00e9duction de l&rsquo;automatisme:<br \/>\n&#8211; \u00e0 l\u2019\u00e9tage auriculaire : bradycardie, diminution de l&rsquo;amplitude et \u00e9largissement de l&rsquo;onde P, bloc sino-atrial voire paralysie sinusale.<br \/>\n&#8211; \u00e0 l\u2019\u00e9tage jonctionnel : bloc AV du 1er ou 2e degr\u00e9.<br \/>\n&#8211; \u00e0 l\u2019\u00e9tage ventriculaire : bloc intraventriculaire (QRS \u00e9largis et crochet\u00e9s ou fragment\u00e9s).<\/li>\n<li>Les ondes T deviennent g\u00e9antes et pointues et le QT se raccourcit.<\/li>\n<\/ol>\n<p>Le potassium (K+)\u00a0 est essentiellement intracellulaire.<br \/>\nLa kali\u00e9mie refl\u00e8te la quantit\u00e9 de potassium extracellulaire.<\/p>\n<p>L&rsquo;ECG traduit les variations du gradient Ki\/Ke (potassium intracellulaire \/ potassium extracellulaire).\u00a0 La morphologie de l&rsquo;onde T \u00e9tant tr\u00e8s variable chez le chien, les augmentations mod\u00e9r\u00e9es de la kali\u00e9mie seront difficilement interpr\u00e9tables.<\/p>\n<p>Le tissu atrial est plus sensible que le tissu ventriculaire \u00e0 l&rsquo;action du potassium. Les modifications ECG porteront d&rsquo;abord sur l&rsquo; activit\u00e9 \u00e9lectrique atriale:<br \/>\n&#8211; diminution de l&rsquo; amplitude de l&rsquo;onde P jusqu&rsquo;\u00e0 sa disparition<br \/>\n&#8211; conduction AV: allongement PR<\/p>\n<p>Les modifications rencontr\u00e9es dans les hyperkali\u00e9mies s\u00e9v\u00e8res (kali\u00e9mie &gt;= 7,5 mmol\/L) sont:<br \/>\n&#8211; disparition de l&rsquo;onde P<br \/>\n&#8211; bradycardie, onde T g\u00e9ante, positive et sym\u00e9trique<br \/>\n&#8211; bloc AV<br \/>\n&#8211; \u00e9largissement des QRS (dysmorphiques par bloc intra-ventriculaire) puis asystolie ou fibrillation ventriculaire.<\/p>\n<p>L&rsquo;acidose, l&rsquo;hyponatr\u00e9mie et l&rsquo;hypocalc\u00e9mie potentialisent les effets d\u2019une hyperkali\u00e9mie.<\/p>\n<p>Les causes d&rsquo;hyperkali\u00e9mie sont nombreuses:<\/p>\n<p style=\"padding-left: 40px;\">&#8211; les maladies r\u00e9nales lorsque la s\u00e9cr\u00e9tion de potassium est diminu\u00e9e (tubulopathie), l&rsquo;hypocorticisme (Addison), les rhabdomyolyses, l&rsquo;h\u00e9molyse, le syndrome urologique f\u00e9lin (avant la lev\u00e9e de l&rsquo;obstacle).<\/p>\n<p style=\"padding-left: 40px;\">&#8211; Les exc\u00e8s d&rsquo;apport ainsi que certains m\u00e9dicaments (inhibiteurs de l&rsquo;enzyme de conversion, sartans,\u00a0 spironolactone,\u00a0 trim\u00e9thoprime) peuvent \u00eatre \u00e0 l&rsquo;origine d&rsquo;hyperkali\u00e9mie.<\/p>\n<figure id=\"attachment_936\" aria-describedby=\"caption-attachment-936\" style=\"width: 521px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-936\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/06\/addison.jpg\" alt=\"Effets de la correction de l'hyperkali\u00e9mie sur le trac\u00e9 ECG\" width=\"521\" height=\"630\" \/><figcaption id=\"caption-attachment-936\" class=\"wp-caption-text\">Trac\u00e9s ECG avant et apr\u00e8s la correction d&rsquo;une hyperkali\u00e9mie<\/figcaption><\/figure>\n<h3><strong>hypokali\u00e9mie:<\/strong><\/h3>\n<p>Elle prolonge la phase 3 du Pa, ce qui peut modifier la repolarisation et favoriser les arythmies.<br \/>\nUne onde U succ\u00e8de parfois \u00e0 l&rsquo;onde T.<\/p>\n<p>Les ondes T+U donnent alors un aspect &lsquo;en S italique couch\u00e9&rsquo; \u00e0 la repolarisation.<\/p>\n<p>Le sous-d\u00e9calage du segment ST s&rsquo;accentue \u00e0 mesure que la kali\u00e9mie diminue.<\/p>\n<p>Le risque de troubles du rythme auriculaire ou ventriculaire existe, en particulier, la torsade de pointes ou la tachycardie ventriculaire bidirectionnelle favoris\u00e9es par un QT long.<\/p>\n<p>Les principales causes d&rsquo;hypokali\u00e9mie sont les pertes digestives ou r\u00e9nales, les diur\u00e9tiques (surtout thiazidiques), les brochodilatateurs \u03b22 agonistes, l&rsquo;insuline et les cortico\u00efdes.<\/p>\n<p>Chez le chat, l&rsquo;hyperaldost\u00e9ronisme d\u00fb \u00e0 une tumeur surr\u00e9nalienne est la cause d&rsquo;hypokali\u00e9mie souvent s\u00e9v\u00e8re et rebelle.<\/p>\n<figure id=\"attachment_953\" aria-describedby=\"caption-attachment-953\" style=\"width: 492px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-953 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/06\/hypokaliemie.jpg\" alt=\"hypokali\u00e9mie au cours d'un hyperaldost\u00e9ronisme chez un chat\" width=\"492\" height=\"402\" \/><figcaption id=\"caption-attachment-953\" class=\"wp-caption-text\">Hypokali\u00e9mie s\u00e9v\u00e8re chez un chat<\/figcaption><\/figure>\n<h3><strong>hypocalc\u00e9mie:<\/strong><\/h3>\n<p>Elle prolonge la phase 2 du Pa.<\/p>\n<p>En pratique les hypocalc\u00e9mies mod\u00e9r\u00e9es ont peu de r\u00e9percussion ECG.<\/p>\n<p>Une hypocalc\u00e9mie s\u00e9v\u00e8re est caract\u00e9ris\u00e9e par un<strong> allongement du segment ST<\/strong> et de l&rsquo;intervalle QT.<\/p>\n<p>Les principales \u00e9tiologies sont l&rsquo;hypoparathyro\u00efdie, l&rsquo;insuffisance r\u00e9nale chronique et les intoxications \u00e0 l&rsquo;\u00e9thyl\u00e8ne glycol.<\/p>\n<h3><strong>hypercalc\u00e9mie:<\/strong><\/h3>\n<p>Elle raccourcit la phase 2 du Pa.<\/p>\n<p>Lors d&rsquo;hypercalc\u00e9mie mod\u00e9r\u00e9e, on note un raccourcissement de l&rsquo;intervalle QT et un sous-d\u00e9calage ST.<\/p>\n<p>Lorsque l&rsquo;hypercalc\u00e9mie s&rsquo;accentue, le segment ST se raccourcit car l&rsquo; onde T commence d\u00e8s la fin du QRS.<\/p>\n<p>Il existe parfois un BAV1.<\/p>\n<p>Le risque rythmique est celui d&rsquo; une tachycardie ventriculaire.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>ECG et perturbations ioniques Les flux d&rsquo; ions (Na+, Ca++ et K+) sont responsables de l&rsquo;activit\u00e9 cardiaque.
