Ce chat de 12 ans est pr\u00e9sent\u00e9 pour abattement marqu\u00e9 et hyporexie.<\/p>\n\n\n\n

L’auscultation r\u00e9v\u00e8le une tachycardie marqu\u00e9e \u00e0 280 bpm.<\/p>\n\n\n\n

Les analyses montrent une hyperthyro\u00efdie (103 nmol\/L) et une hypokali\u00e9mie profonde (2,5 millimoles\/litre)<\/p>\n\n\n\n

A premi\u00e8re vue, l’onde P parait exag\u00e9r\u00e9ment large alors que l’\u00e9chocardiographie met en \u00e9vidence un \u00e9paississement mod\u00e9r\u00e9 des parois ventriculaires gauches sans dilatation atriale.<\/p>\n\n\n\n

En fait il s’agit d’une onde U d’hypokali\u00e9mie qui fait suite \u00e0 une onde T classique et les ondes P ne sont pas visibles.<\/p>\n\n\n\n

24 heures plus tard apr\u00e8s perfusion de K+, la kali\u00e9mie s’est normalis\u00e9e, le rythme est redevenu sinusal et l’onde U a disparu.<\/p>\n\n\n\n

L’ECG montre alors un aspect classique d’ hyperthyro\u00efdie accompagn\u00e9e de CMH avec une tachycardie encore marqu\u00e9e, un \u00e9largissement ainsi qu’une augmentation d’amplitude des QRS. On notera \u00e9galement la pr\u00e9sence d’un sous-d\u00e9calage ST.<\/p>\n\n\n\n
