L’\u00e9chocardiographie de ce chat montre une cardiomyopathie hypertrophique aux retentissements peu marqu\u00e9s. Cependant plusieurs \u00e9pisodes syncopaux sont rapport\u00e9s.<\/p>\n

On remarque en premier lieu, de nombreuses extrasystoles ventriculaires tr\u00e8s pr\u00e9coces.<\/p>\n

Le d\u00e9but de la d\u00e9polarisation extrasystolique se produit \u00e0 la fin de l’onde T (p\u00e9riode vuln\u00e9rable) et est \u00e0 m\u00eame de donner naissance \u00e0 une tachycardie ventriculaire, voire une fibrillation ventriculaire.<\/p>\n

D’autre part, il faut pr\u00eater une attention particuli\u00e8re au dernier complexe QRS. En effet , celui-ci se pr\u00e9sente comme un complexe extrasystolique mais de polarit\u00e9 et de morphologie diff\u00e9rentes.<\/p>\n

Plut\u00f4t que d’\u00e9voquer plusieurs foyers arythmog\u00e8nes sur ce trac\u00e9, il faut remarquer que ce complexe pr\u00e9coce survient apr\u00e8s un cycle long.<\/p>\n

L’alternance cycle long – cycle court favorise les troubles de conduction intraventriculaire car la dur\u00e9e de la p\u00e9riode r\u00e9fractaire du faisceau de His est proportionnelle \u00e0 la dur\u00e9e de l’intervalle RR qui pr\u00e9c\u00e8de.<\/p>\n

Lorsqu’un influx pr\u00e9coce survient alors que les cellules du faisceau de His ne sont que partiellement sorties de leur p\u00e9riode r\u00e9fractaire, il s’ensuit un ralentissement de la conduction intraventriculaire qui donne cet aspect de QRS \u00e9largi \u00e0 l’allure extrasystolique alors qu’il s’agit d’un complexe d’origine sinusale. C’est le ph\u00e9nom\u00e8ne d’Ashman.<\/p>\n
