{"id":2830,"date":"2019-12-02T23:07:32","date_gmt":"2019-12-02T22:07:32","guid":{"rendered":"http:\/\/vet-ecg.ddns.net\/?page_id=2830"},"modified":"2020-02-03T09:43:53","modified_gmt":"2020-02-03T08:43:53","slug":"faisceaux-accessoires","status":"publish","type":"page","link":"http:\/\/vet-ecg.ddns.net\/index.php\/faisceaux-accessoires\/","title":{"rendered":"Faisceau accessoire"},"content":{"rendered":"<p>Les faisceaux accessoires sont des r\u00e9seaux de fibres musculaires pr\u00e9sentes \u00e0 la naissance qui peuvent, dans certaines circonstances, se comporter comme une voie de conduction suppl\u00e9mentaire.<\/p>\n<p>Il existe plusieurs types de faisceaux accessoires:<\/p>\n<ul>\n<li>Les fibres de Kent (atrio-ventriculaires)<\/li>\n<li>Les fibres de James (atrio-nodales)<\/li>\n<li>Les fibres de Maha\u00efm (nodo-ventriculaires)<\/li>\n<\/ul>\n<p>Lorsque un faisceau de fibres devient perm\u00e9able, il shunte les voies de conductions nodo-hissiennes habituelles.<\/p>\n<p>On parle dans ce cas de syndrome de pr\u00e9excitation ventriculaire lorsqu&rsquo; une partie du myocarde ventriculaire est d\u00e9polaris\u00e9 via le faisceau accessoire, tandis que le reste des ventricules est d\u00e9polaris\u00e9 depuis les voies de conductions habituelles.<\/p>\n<p>Le sens de conduction est g\u00e9n\u00e9ralement r\u00e9trograde: L&rsquo;influx descend par les voies nodo-hissiennes et remontent par le faisceau (conduction orthodromique, ECG normal en rythme sinusal).<\/p>\n<p>Parfois la conduction s&rsquo;effectue de mani\u00e8re ant\u00e9rograde: l&rsquo;influx descend par le faisceau et remonte par les voies de conduction nodo-hissiennes (conduction antidromique, ECG avec onde delta).<\/p>\n<p>Le danger d&rsquo;une conduction accessoire est li\u00e9 \u00e0 la p\u00e9riode r\u00e9fractaire du faisceau.
