{"id":2762,"date":"2019-11-30T10:11:33","date_gmt":"2019-11-30T09:11:33","guid":{"rendered":"http:\/\/vet-ecg.ddns.net\/?page_id=2762"},"modified":"2019-11-30T11:24:52","modified_gmt":"2019-11-30T10:24:52","slug":"lecture-dun-ecg-3","status":"publish","type":"page","link":"http:\/\/vet-ecg.ddns.net\/index.php\/lecture-dun-ecg-3\/","title":{"rendered":"Lecture d&rsquo;un ECG (3)"},"content":{"rendered":"<h3>Axe de QRS:<\/h3>\n<p>L&rsquo;axe des QRS correspond \u00e0 l&rsquo;axe du champ \u00e9lectrique g\u00e9n\u00e9r\u00e9 par les cellules cardiaques.<br \/>\nIl est la r\u00e9sultante de l&rsquo;ensemble des vecteurs de d\u00e9polarisation des ventricules droit et gauche. La masse du ventricule gauche (VG) \u00e9tant pr\u00e9pond\u00e9rante, l&rsquo;axe \u00e9lectrique &lsquo;normal&rsquo; correspond \u00e0 l&rsquo;axe du c\u0153ur (environ 60\u00b0).<br \/>\nL&rsquo;axe \u00e9lectrique repr\u00e9sente la direction et le sens g\u00e9n\u00e9ral de la d\u00e9polarisation des ventricules.<br \/>\nSi le VG est hypertrophi\u00e9, l&rsquo;axe sera d\u00e9vi\u00e9 \u00e0 gauche (inf\u00e9rieur \u00e0 40\u00b0 chez le chien).<br \/>\nPour que le ventricule droit (VD) entraine une d\u00e9viation axiale droite (sup\u00e9rieure \u00e0 100\u00b0) il faut que le VD soit tr\u00e8s hypertrophi\u00e9 compte-tenu de sa masse relativement faible par rapport \u00e0 celle du VG.<\/p>\n<p><strong>Chez les chiens, l&rsquo;axe doit \u00eatre compris entre 40\u00b0 et 100\u00b0.<\/strong><\/p>\n<p>En simplifiant, on peut dire que:<br \/>\n&#8211; chez les chats, l&rsquo;axe doit \u00eatre compris entre 0\u00b0 et 160\u00b0.<\/strong><\/p>\n<p>D1 peut \u00eatre n\u00e9gatif mais AVF positif.<\/p>\n<figure id=\"attachment_696\" aria-describedby=\"caption-attachment-696\" style=\"width: 1154px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-696 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/AXE-QRS-ct3-1.jpg\" alt=\"L'axe habituel de QRS chez le chat est compris entre 0\u00b0 et 160\u00b0\" width=\"1154\" height=\"905\"><figcaption id=\"caption-attachment-696\" class=\"wp-caption-text\">Axe \u00e9lectrique du QRS chez le chat<\/figcaption><\/figure>\n<p>L&rsquo;axe de QRS se trouve entre les 2 d\u00e9rivations o\u00f9 l&rsquo;amplitude de QRS (somme alg\u00e9brique des d\u00e9flexions positives et n\u00e9gatives) est la plus ample ou par la perpendiculaire \u00e0 la d\u00e9rivation o\u00f9 son amplitude est nulle ou iso-diphasique (somme alg\u00e9brique de la polarit\u00e9 positive et de la polarit\u00e9 n\u00e9gative = 0).<\/p>\n<p>Par exemple, si les&nbsp; amplitudes les plus fortes se trouvent en <strong>D2<\/strong> (60\u00b0) et <strong>D3<\/strong> (120\u00b0) et sont \u00e9quivalentes, on en conclut que l&rsquo;axe de QRS est approximativement \u00e0 (60+120)\/2 = 90\u00b0.<\/p>\n<figure id=\"attachment_398\" aria-describedby=\"caption-attachment-398\" style=\"width: 252px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-398\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/04\/QRS-valeur.jpg\" alt=\"somme alg\u00e9brique des d\u00e9flexions positives et  n\u00e9gatives: -10 + 23 = 13 mm\" width=\"252\" height=\"295\"><figcaption id=\"caption-attachment-398\" class=\"wp-caption-text\">Calcul de l&rsquo;amplitude de QRS permettant la d\u00e9termination de l&rsquo;axe de QRS (\u00c2QRS)<\/figcaption><\/figure>\n<p>La notion d&rsquo;axe de QRS (que l&rsquo;on note \u00c2QRS) est moins pertinente chez le chien et le chat que chez l&rsquo;homme car le c\u0153ur n&rsquo;est pas au centre d&rsquo;un triangle \u00e9quilat\u00e9ral (triangle d&rsquo;Einthoven) chez qui les sommets correspondent aux membres ant\u00e9rieurs et au membre post\u00e9rieur gauche.<\/p>\n<p>Autre m\u00e9thode de calcul de l&rsquo;axe de QRS:<\/p>\n<figure id=\"attachment_729\" aria-describedby=\"caption-attachment-729\" style=\"width: 1575px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-729 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/AQRS-calcul3.jpg\" alt=\"repr\u00e9sentation graphique de l'axe de QRS\" width=\"1575\" height=\"1467\"><figcaption id=\"caption-attachment-729\" class=\"wp-caption-text\">report des valeurs des amplitudes de QRS mesur\u00e9es en D1 et AVF<\/figcaption><\/figure>\n<p>Ainsi, chez le chien et le chat, un axe&nbsp; pourra \u00eatre normal malgr\u00e9 un remodelage marqu\u00e9 de la silhouette cardiaque, en revanche,&nbsp; une d\u00e9viation axiale devra inciter \u00e0 rechercher une cardiom\u00e9galie, un bloc de conduction (h\u00e9mibloc) ou une masse thoracique d\u00e9pla\u00e7ant le coeur.<\/p>\n<h3><strong>R\u00e9sum\u00e9 de la d\u00e9marche:<\/strong><\/h3>\n<p><strong>A)<\/strong> <strong>&#8211; Appr\u00e9cier la qualit\u00e9 du trac\u00e9:<\/strong> La stabilit\u00e9 de la ligne de base conditionne la fiabilit\u00e9 des r\u00e9sultats.<br \/>\n&#8211; V\u00e9rification de l&rsquo;\u00e9talonnage (25 mm\/s et 10 mm\/mV).<br \/>\n&#8211; V\u00e9rification du profil de l&rsquo;animal: race, morphotype, \u00e2ge, poids, traitement en cours, \u00e9tat clinique (fi\u00e8vre, dysthyro\u00efdies, an\u00e9mie, dyspn\u00e9e, choc), niveau de stress ou de douleur.<\/p>\n<p><strong>B)<\/strong> <strong>Examen de l&rsquo;activit\u00e9 atriale:<\/strong><br \/>\n&#8211; Appr\u00e9cier l&rsquo;origine sinusale: dur\u00e9e constante de PQ, polarit\u00e9 de l&rsquo;onde P positive en D1, et D2.<br \/>\n&#8211; Appr\u00e9cier la fr\u00e9quence, l&rsquo;amplitude, la dur\u00e9e et l&rsquo;aspect des ondes P et la r\u00e9gularit\u00e9 des intervalles PP.<\/p>\n<p><strong>C)<\/strong> <strong>Examen de l&rsquo;activit\u00e9 ventriculaire:<\/strong><br \/>\n&#8211; Examiner la r\u00e9gularit\u00e9 des intervalles RR<br \/>\n&#8211; Examiner la dur\u00e9e, l&rsquo;amplitude et la morphologie des QRS<br \/>\n&#8211; Appr\u00e9cier l&rsquo;axe des QRS (D1 et AVF positifs)<\/p>\n<p><strong>D) Examen de la synchronisation de l&rsquo;activit\u00e9 atriale et de l&rsquo;activit\u00e9 ventriculaire:<\/strong><br \/>\n&#8211; Intervalle PQ excluant un bloc (PQ long) ou un syndrome de pr\u00e9-excitation (PQ court).<br \/>\n&#8211; Autant d&rsquo;ondes P que de QRS<br \/>\n&#8211; Recherche de complexes ectopiques (Extra-systoles, battement d&rsquo;\u00e9chappement)<\/p>\n<p><strong>E) Examen de la repolaristation:<\/strong><br \/>\n&#8211; Segment ST iso\u00e9lectrique, d\u00e9viation minime du point J.<br \/>\n&#8211; Examen de l&rsquo;onde T: amplitude par rapport \u00e0 celle du QRS, morphologie.<br \/>\n&#8211; Examen de la dur\u00e9e de la repolarisation (QT)<\/p>\n<p><strong>F)<\/strong> <strong>R\u00e9daction du compte-rendu:<\/strong><br \/>\n&#8211; Nature du rythme et fr\u00e9quence cardiaque.<br \/>\n&#8211; Appr\u00e9ciation de l&rsquo;arythmie sinusale respiratoire<br \/>\n&#8211; Mention des \u00e9ventuels changements de rythme (extr-systoles, pauses)<br \/>\n&#8211; Appr\u00e9ciation de la conduction AV (PQ) et intraventriculaire (morphologie des QRS)<br \/>\n&#8211; Appr\u00e9ciation de la repolarisation<\/p>\n<p><strong>G) Conclusion:<\/strong><\/p>\n<p>&#8211;
