{"id":275,"date":"2018-03-28T09:57:02","date_gmt":"2018-03-28T07:57:02","guid":{"rendered":"http:\/\/192.168.1.104\/?page_id=275"},"modified":"2019-11-30T10:34:33","modified_gmt":"2019-11-30T09:34:33","slug":"lecture-dun-ecg-les-bases","status":"publish","type":"page","link":"http:\/\/vet-ecg.ddns.net\/index.php\/lecture-dun-ecg-les-bases\/","title":{"rendered":"Lecture d&rsquo;un ECG (1)"},"content":{"rendered":"<h3><strong>Comment lire un ECG<\/strong><\/h3>\n<p><strong>L\u2019interpr\u00e9tation se fera toujours en tenant compte du contexte<\/strong>:<\/p>\n<p>\u2013 format du chien (br\u00e9viligne, longiligne): tonus neurov\u00e9g\u00e9tatif diff\u00e9rent.<\/p>\n<p>\u2013 race : \u2013 sharpe\u00ef (diminution d\u2019amplitude des signaux li\u00e9e aux plis de peau)<\/p>\n<p style=\"padding-left: 60px;\">\u2013 chihuahua: onde T positive en V10.<\/p>\n<p style=\"padding-left: 60px;\">\u2013 boxer, doberman, berger allemand, schnauzer nain, labrador, golden retriever<\/p>\n<p style=\"padding-left: 60px;\">\u2013 l\u00e9vriers: augmentation d\u2019amplitude des signaux (morphologie de la cage thoracique).<\/p>\n<p style=\"padding-left: 60px;\">\u2013 brachyc\u00e9phales: majoration de l\u2019arythmie respiratoire, M\u00f6bitz 2, li\u00e9s au tonus vagal.<\/p>\n<p>&#8211; niveau de stress, de douleur, \u00e9tat neurologique.<\/p>\n<p>&#8211; statut pond\u00e9ral (maigreur, ob\u00e9sit\u00e9): modifie l&rsquo;amplitude des signaux.<\/p>\n<p>&#8211; th\u00e9rapeutique en cours, m\u00eame non cardiaque: neuroleptiques, antid\u00e9presseurs, digoxine, anti-arytmiques, b\u00eatabloquants. Ces mol\u00e9cules engendrent des modifications diverses de l&rsquo;ECG:\u00a0 acc\u00e9l\u00e9ration ou d\u00e9c\u00e9l\u00e9ration de la FC, augmentation de la dur\u00e9e de QRS, troubles de la conduction: BAV, dysrythmie.<\/p>\n<p>&#8211; \u00e9tat clinique (fi\u00e8vre, dysthyro\u00efdies, an\u00e9mie, dyspn\u00e9e, polypn\u00e9e, choc)<\/p>\n<p>&#8211; et surtout taille et poids de l&rsquo;animal: estimation de la FC et de la dur\u00e9e de QRS attendues.<\/p>\n<p><span style=\"color: #000000;\">\u00a0<\/span>Sur des chiens indemnes de cardiopathie, la dur\u00e9e, et l&rsquo;amplitude des QRS ainsi que la fr\u00e9quence cardiaque seront tr\u00e8s diff\u00e9rentes selon que l&rsquo;on examine un chihuahua de 1,5 kg ou un saint bernard de 85 kg.<\/p>\n<h3><strong>Reconnaissance des diff\u00e9rentes d\u00e9flexions:<\/strong><\/h3>\n<figure id=\"attachment_708\" aria-describedby=\"caption-attachment-708\" style=\"width: 290px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-708\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/cycles-cardiaques.jpg\" alt=\"systole auriculaire (P), systole ventriculaire (QRS) et repolarisation (T)\" width=\"290\" height=\"245\" \/><figcaption id=\"caption-attachment-708\" class=\"wp-caption-text\">un cycle cardiaque complet<\/figcaption><\/figure>\n<p>Une d\u00e9flexion est une onde positive ou n\u00e9gative qui est l&rsquo;expression d&rsquo;un vecteur \u00e9lectrique de d\u00e9polarisation ou de repolarisation.<\/p>\n<p>La d\u00e9polarisation d&rsquo;un myocyte correspond \u00e0 l&rsquo;ouverture des canaux calciques .<br \/>Le potentiel membranaire passe alors de -40 mV \u00e0 0 mV.<br \/>La repolarisation correspond \u00e0 la fermeture des canaux Ca++ et \u00e0 l&rsquo;ouverture des canaux K+\u00a0 jusqu&rsquo;\u00e0 ce que le potentiel atteigne -60 mV.<br \/>A ce moment les canaux Na+ (lents) s&rsquo;ouvrent jusqu&rsquo;au seuil d&rsquo;excitation de -40 mV. Un nouveau cycle recommence.<\/p>\n<p><span style=\"color: #000000;\"><span style=\"color: #ff0000;\">onde P:<\/span> d\u00e9flexion (positive en D1 et D2) qui pr\u00e9c\u00e8de le complexe QRS en rythme sinusal.<br \/><span style=\"color: #ff0000;\">onde Q:<\/span> premi\u00e8re d\u00e9flexion n\u00e9gative du complexe QRS, elle peut \u00eatre absente mais doit toujours \u00eatre fine et peu ample.<br \/><span style=\"color: #ff0000;\">onde R:<\/span> premi\u00e8re d\u00e9flexion positive du complexe QRS, sa plus forte amplitude doit \u00eatre comprise entre 2 et 3,5 mV selon la taille de l&rsquo;animal.<br \/><span style=\"color: #ff0000;\">onde S:<\/span> d\u00e9flexion n\u00e9gative qui suit l&rsquo;onde R. Elle est souvent absente ou de tr\u00e8s faible amplitude en D1 et D2.<br \/><span style=\"color: #ff0000;\">onde T:<\/span> d\u00e9flexion qui peut \u00eatre positive ou n\u00e9gative mais asym\u00e9trique et d&rsquo;amplitude inf\u00e9rieure au quart de l&rsquo;onde R.<br \/><span style=\"color: #ff0000;\">onde U:<\/span> sa pr\u00e9sence est rare et g\u00e9n\u00e9ralement physiologique tant que son amplitude est faible et inf\u00e9rieure \u00e0 celle de l&rsquo;onde T. Sa signification n&rsquo;est pas bien connue. Une amplitude sup\u00e9rieure \u00e0 celle de l&rsquo;onde T peut \u00eatre le reflet d&rsquo;une hypokali\u00e9mie.<\/span><\/p>\n<p>Sch\u00e9matiquement on peut dire que l&rsquo;onde P correspond \u00e0 la systole auriculaire, le QRS \u00e0 la systole ventriculaire et l&rsquo;onde T \u00e0 la repolarisation des ventricules.<\/p>\n<p>Par convention une d\u00e9flexion d&rsquo;amplitude inf\u00e9rieure \u00e0 0,5 mV s&rsquo;\u00e9crit avec une minuscule, au del\u00e0 de 0,5 mV on l&rsquo;\u00e9crit avec une majuscule.<\/p>\n<figure id=\"attachment_643\" aria-describedby=\"caption-attachment-643\" style=\"width: 843px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-643 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/QRS-morpho2.jpg\" alt=\"lettre minuscule pour les d\u00e9flexions &lt;= 5 mm (\u00e0 10 mm\/mV)\" width=\"843\" height=\"366\" \/><figcaption id=\"caption-attachment-643\" class=\"wp-caption-text\">Les diff\u00e9rents aspects morphologiques de QRS<\/figcaption><\/figure>\n<h3><span style=\"color: #000000;\"><strong>La notion de rythme cardiaque<\/strong><\/span><\/h3>\n<p style=\"text-align: left;\">C&rsquo;est une notion complexe qui inclut:<br \/>&#8211; la r\u00e9gularit\u00e9 de la fr\u00e9quence cardiaque.<br \/>&#8211; la r\u00e9gularit\u00e9 de la succession des d\u00e9flexions P-QRS-T.<br \/>&#8211; le rapport chronologique entre la contraction atriale et la contraction ventriculaire.<br \/>&#8211; la synchronisation de ces contractions.<\/p>\n<p>Le rythme cardiaque physiologique (rythme sinusal) est celui qui permet d&rsquo;obtenir une \u00e9jection optimale de la masse sanguine.<\/p>\n<p>\u00a0<\/p>\n<h3><span style=\"color: #000000;\"><strong>L&rsquo;ECG\u00a0 normal <\/strong><\/span><\/h3>\n<h3><strong>Premi\u00e8re \u00e9tape:\u00a0 le trac\u00e9 est-il sinusal?<\/strong><\/h3>\n<figure id=\"attachment_783\" aria-describedby=\"caption-attachment-783\" style=\"width: 970px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-783 size-full\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/2018\/05\/cycle-PQRST-33.jpg\" alt=\"caract\u00e9ristiques d'un cycle cardiaque chez le chien\" width=\"970\" height=\"489\" \/><figcaption id=\"caption-attachment-783\" class=\"wp-caption-text\">valeurs usuelles chez le chien et le chat<\/figcaption><\/figure>\n<h2><strong>D\u00e9finition du rythme sinusal:<\/strong><\/h2>\n<p><strong>1)<\/strong> Un rythme sinusal est compos\u00e9 exclusivement d&rsquo;une succession de s\u00e9quences P-QRS-T\u00a0 \u00e0 intervalles r\u00e9guliers.<\/p>\n<p><strong>2)<\/strong> Pour \u00eatre sinusal, le complexe P-QRS-T\u00a0 qui prend naissance dans le n\u0153ud sinusal doit emprunter les voies de conduction d\u00e9di\u00e9es:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>n\u0153ud sinusal<\/li>\n<li>n\u0153ud atrioventriculaire<\/li>\n<li>tronc du faisceau de His<\/li>\n<li>les 2 branches du faisceau de His<\/li>\n<li>le r\u00e9seau de Purkinje<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>3)<\/strong> Par ailleurs, la propagation de cet influx doit se faire sans ralentissement ni acc\u00e9l\u00e9ration\u00a0 (hormis l&rsquo;influence de la respiration) sur son trajet (bloc, faisceau accessoire) afin de maintenir une parfaite synchronisation des \u00e9v\u00e8nements.<\/p>\n<p>La polarit\u00e9 de l&rsquo;onde P doit \u00eatre positive dans les d\u00e9rivations D1, D2, D3, AVF et n\u00e9gative en AVR. Une polarit\u00e9 n\u00e9gative en D2 doit faire suspecter un rythme atrial et une polarit\u00e9 n\u00e9gative de P en D1 \u00e9voque une inversion d&rsquo;\u00e9lectrodes.<\/p>\n<p>Les dur\u00e9es de l&rsquo;
