L&rsquo;onde T est de polarit\u00e9 oppos\u00e9e \u00e0 celle du QRS et le segment ST est mal individualis\u00e9. La fr\u00e9quence est g\u00e9n\u00e9ralement >150 bpm.<br>Le diagnostic est pos\u00e9 lorsqu&rsquo;on met en \u00e9vidence une dissociation atrioventriculaire et des complexes de capture ou de fusion.<\/p>\n\n\n\n<p>Lorsque la polarit\u00e9 des QRS est n\u00e9gative en D2, l&rsquo;origine de la tachycardie est ventriculaire gauche. Elle est ventriculaire droite si la polarit\u00e9 est positive.<\/p>\n\n\n\n<p><strong>Diagnostic diff\u00e9rentiel<\/strong> <strong>TV\/TSV avec bloc de branche:<\/strong><\/p>\n\n\n\n<p>La dissociation atrioventriculaire est rarement visible car les ondes P sont g\u00e9n\u00e9ralement dissimul\u00e9es dans les QRS.  Cela rend souvent le diagnostic difficile. <\/p>\n\n\n\n<p>On peut parfois deviner les ondes P lorsqu&rsquo;elles perturbent de mani\u00e8re al\u00e9atoire la morphologie d&rsquo;un QRS ou d&rsquo;une onde T. <\/p>\n\n\n\n<p>D&rsquo;autre part, si aucun complexe de fusion ou de capture n&rsquo;est visible, il devient difficile de diff\u00e9rencier une tachycardie ventriculaire d&rsquo;une tachycardie supraventriculaire (TSV) avec bloc de branche.<\/p>\n\n\n\n<p>Une TV prend naissance en dessous de la bifurcation du faisceau de His.     Si celle-ci est issue du septum interventriculaire, on parle de TV septale. <\/p>\n\n\n\n<p>Dans ce cas, les QRS sont moins larges (50 \u00e0 70 ms) et moins volt\u00e9s que dans une TV droite ou gauche. L&rsquo;onde T des tachycardies septales peut \u00eatre biphasique.<\/p>\n\n\n\n<p> Il est important de distinguer TV et TSV. En effet,  la tachycardie ventriculaire peut se transformer  en fibrillation ventriculaire, rapidement mortelle.<\/p>\n\n\n\n<p>En m\u00e9decine humaine, il existe de nombreux indices qui permettent de distinguer TV et TSV. <\/p>\n\n\n\n<p>Dans la pratique v\u00e9t\u00e9rinaire courante, il est difficile de reproduire ces indices qui reposent souvent sur l&rsquo;interpr\u00e9tation des d\u00e9rivations pr\u00e9cordiales. <\/p>\n\n\n\n<p>Cependant, il est possible de s&rsquo;inspirer de certains de ces indices (indices de Verecke\u00ef et de Pava).<\/p>\n\n\n\n<p><strong>Indices plaidant en faveur d&rsquo;une TV:<\/strong><\/p>\n\n\n<ul>\n<li>Une onde R initiale en AVR<\/li>\n<li>Une onde R ou Q &gt; 20 ms en AVR<\/li>\n<li>Une dur\u00e9e RS &gt; 50 ms<\/li>\n<li>Une augmentation de la d\u00e9flexion intrins\u00e9co\u00efde (temps d&rsquo;inscription de l&rsquo;onde R) &gt; 30 ms en D2<\/li>\n<li>Un crochetage de la pente initiale d&rsquo;un QRS n\u00e9gatif<\/li>\n<li>Un axe de QRS compris entre 180\u00b0 et 270\u00b0<\/li>\n<li>Une amplitude initiale inf\u00e9rieure \u00e0 l&rsquo;amplitude terminale du QRS<\/li>\n<\/ul>\n\n\n<p><\/p>\n\n\n\n<figure class=\"wp-block-image\"><img loading=\"lazy\" decoding=\"async\" width=\"1560\" height=\"1200\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/d\u00e9tails-QRS-2-screenshot-3.jpg\" alt=\"\" class=\"wp-image-2164\"\/><figcaption>d\u00e9tails ratio de v\u00e9locit\u00e9<\/figcaption><\/figure>\n\n\n\n<figure class=\"wp-block-image\"><img loading=\"lazy\" decoding=\"async\" width=\"750\" height=\"908\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/d\u00e9tails-QRS-1-2.jpg\" alt=\"\" class=\"wp-image-2168\"\/><figcaption>temps d&rsquo;inscription de l&rsquo;onde R ou Q en D2<\/figcaption><\/figure>\n","protected":false},"excerpt":{"rendered":"<p>Des QRS amples , larges (70 ms) et souvent monomorphes caract\u00e9risent les tachycardie ventriculaire (TV).
