{"id":1639,"date":"2019-05-10T11:39:51","date_gmt":"2019-05-10T09:39:51","guid":{"rendered":"http:\/\/192.168.1.104\/?page_id=1639"},"modified":"2019-11-07T17:41:57","modified_gmt":"2019-11-07T16:41:57","slug":"blocs-de-branches-generalites","status":"publish","type":"page","link":"http:\/\/vet-ecg.ddns.net\/index.php\/blocs-de-branches-generalites\/","title":{"rendered":"Blocs de branches: g\u00e9n\u00e9ralit\u00e9s"},"content":{"rendered":"\n<p>Les blocs de branches (BB) correspondent \u00e0 une interruption (bloc complet) ou \u00e0 un ralentissement (bloc incomplet) de la conduction dans les branches du faisceau de His.<\/p>\n\n\n\n<p>Le faisceau de His se compose d&rsquo;une branche droite et d&rsquo;une branche gauche.<\/p>\n\n\n\n<p>La branche gauche se ramifie en 2 h\u00e9mibranches (ant\u00e9rieure et post\u00e9rieure). Il est possible de distinguer \u00e0 l\u2019ECG, un trouble affectant seulement une h\u00e9mibranche.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<figure class=\"wp-block-image\"><img loading=\"lazy\" decoding=\"async\" width=\"640\" height=\"552\" src=\"http:\/\/vet-ecg.ddns.net\/wp-content\/uploads\/voies-de-conduction-cn3.jpg\" alt=\"\" class=\"wp-image-2053\"\/><\/figure>\n\n\n\n<p>Anciennement appel\u00e9 h\u00e9mibloc ant\u00e9rieur ou post\u00e9rieur gauche, on pr\u00e9f\u00e8re aujourd&rsquo;hui l&rsquo;appellation bloc fasciculaire ant\u00e9rieur ou post\u00e9rieur gauche.<\/p>\n\n\n\n<p>Selon le contexte, ces blocs peuvent \u00eatre&nbsp;: <\/p>\n\n\n\n<p><strong>fonctionnels<\/strong>: Ils apparaissent si un influx rencontre sur son chemin des cellules qui ne sont pas encore sorties de leur p\u00e9riode r\u00e9fractaire. Ils sont g\u00e9n\u00e9ralement transitoires et peuvent \u00eatre alternants (tant\u00f4t branche droite, tant\u00f4t branche gauche sur un m\u00eame trac\u00e9).<\/p>\n\n\n\n<p><strong>organiques<\/strong>: Une l\u00e9sion du tissu de conduction est g\u00e9n\u00e9ralement en cause. Ils se rencontrent lors de pathologie \u00e9volu\u00e9e sont g\u00e9n\u00e9ralement permanents.<\/p>\n\n\n\n<p>Un bloc de branche complet entraine un asynchronisme dans la d\u00e9polarisation des deux ventricules.<\/p>\n\n\n\n<p>Un BB  s&rsquo;associe souvent mais pas n\u00e9cessairement \u00e0 un rythme sinusal. Il peut se rencontrer lors de troubles du rythme supraventriculaire mais il n\u2019est pas possible de le diagnostiquer lors de rythme ventriculaire.<\/p>\n\n\n\n<p>De plus, on peut rencontrer sur un m\u00eame trac\u00e9, un bloc de branche  et un autre trouble de la conduction, par exemple, un BAV du 1er degr\u00e9 ou encore un bloc de branche droit et un h\u00e9mibloc gauche.<\/p>\n\n\n\n<p>Lors de bloc de branche, on ne peut pas calculer l\u2019axe de QRS par les m\u00e9thodes conventionnelles. <\/p>\n\n\n<h4>Caract\u00e9ristiques d&rsquo;un bloc de branche<\/h4>\n\n\n<ul class=\"wp-block-list\"><li>Un bloc complet entraine un \u00e9largissement du QRS &gt; 70 ms.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Les QRS sont amples et l\u2019onde T est invers\u00e9e par rapport \u00e0 la polarit\u00e9 dominante du QRS.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Un bloc d\u2019une h\u00e9mibranche a pour cons\u00e9quence un \u00e9largissement moindre du QRS (50 \u00e0 70 ms).<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>En rythme sinusal, les ondes P et &nbsp;les intervalles PQ sont normaux.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Lors de BB droit, le ventricule gauche active  le ventricule droit qui est donc d\u00e9polaris\u00e9 avec retard.<\/li><li>La polarit\u00e9 du QRS est donc invers\u00e9e.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Lors de BB gauche, la polarit\u00e9 du QRS n&rsquo;est pas modifi\u00e9e, le diagnostic se fera sur la dur\u00e9e de QRS.<\/li><\/ul>\n\n\n\n<p><strong>Les diagnostics diff\u00e9rentiels porteront sur<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Une hypertrophie ventriculaire \u00e0 l\u2019origine d\u2019 un \u00e9largissement mod\u00e9r\u00e9 du QRS. Une radiographie ou une \u00e9chocardiographie sera n\u00e9cessaire pour faire le diagnostic surtout en cas d\u2019 h\u00e9mibloc.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Un trouble de la conduction non sp\u00e9cifique li\u00e9 \u00e0 la prise d\u2019antiarythmiques de type stabilisants de membrane.<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Une pr\u00e9excitation ventriculaire due \u00e0 un faisceau accessoire.<\/li><\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Les blocs de branches (BB) correspondent \u00e0 une interruption (bloc complet) ou \u00e0 un ralentissement (bloc incomplet) de la conduction dans les branches du faisceau de His.
